Please contact our office with pertinent medical information including, but not limited to:
- Patient’s full name
- Date of Birth
- Social Security Number
- Phone number
- Valid email address
- Active Insurance Policies and policy numbers
- Reason for referral
While on the phone, we will provide you with appointment options based on your patients needs. Patient’s medical records may be securely faxed to (731) 784-4308 or emailed to [email protected].
Please also include the referring physician’s name and contact information in your fax or email so that we may keep the referring practice informed of treatment plans.
If you are scheduling Mohs surgery for your patient:

You may reach our office manager and billing supervisor, Jeniffer, at ext. 103, or one of our triage nurses at ext. 107 to schedule your patient for surgery. Please have your patient’s pathology report faxed to (731) 784-4308 or securely emailed to [email protected] prior to their scheduled surgery.
